The following requirements are to be read and signed by each parent. Failure to comply with these team policies could cause your child to not play the full game each week. Before signing please understand these policies. This form will be kept on file with the Mesquite Soccer Association.
- Players
Attendance:
in order to play the full game each week
- each player must attend the practices
during the prior week. This will be strictly
enforced this season. Not attending practices
affects not only your child but also the
rest of the team. If you are not able to
attend a practice, please call me and inform
me of the reason for the absence.
- Game
Attendance:
please have the girls be in attendance for
the full game unless a prior arrangement
has been made. Please arrive at the field
at least 25 minutes before the scheduled
game time.
- Parents
Attendance:
at least one parent or guardian must attend
the full game (unless a prior arrangement
has been made).
- Jewelry:
each player should remove all jewelry before
practices and games. Jewelry could injure
another player and also the player wearing
the jewelry. Referees should require players
to remove all jewelry so let's get in a
habit at practices also.
- Equipment
Requirements:
- shin guards
(without shin guards a player cannot
practice or play)
- appropriate
shoes for running in
- size 3
soccer ball(for girls to bring to practice
and play with at home)
- uniforms
for game time
- Registration
Cost & a Copy of a Birth Certificate:
these will be due to me before the first
game. I will provide everyone with a deadline
for these items.
- Health
Issues:
please inform me prior to the first practice
of any health related issues that would
could have an adverse effect on your daughter
running.
- Doctor's
Notes:
if your daughter has been injured (ie. broken
bone, etc) a doctor's note will be required
for your daughter to practice/play.
- Game
Behavior:
- Excessive
yelling will not be allowed!
- If your
daughter gets hurt during a game, I
will enter the field of play when permitted
by the referee. If the injury is severe,
the parent(s) will be asked to come
onto the field immediately.
- Excessive
verbal abuse to a referee with not be
tolerated! You will be asked to leave
the field if your verbal tone is not
corrected.
